Medical Billing Strategies for Family Medicine Practices

By the HealthRescue Partners team Updated
Medical Billing Strategies for Family Medicine Practices

Family medicine runs on high volume and relatively modest reimbursement per visit, so small billing gaps add up quickly. These strategies help primary care practices collect what they've earned.

1. Preventive and problem visits on the same day

When a patient comes for a physical and also raises a new or worsening problem that needs significant, separate work, both services can often be billed: the preventive visit and a problem-oriented E/M visit with modifier 25. Documentation must clearly show the separate problem work. Patients may owe cost-sharing for the problem visit, so explain this up front.

2. Medicare annual wellness visits

The Medicare Annual Wellness Visit is different from a physical exam and has its own requirements and codes. Many practices under-schedule it. A recall process for eligible Medicare patients adds revenue and improves preventive care.

3. Care management services

Chronic care management, transitional care management after hospital discharge, and remote patient monitoring are paid services with specific consent, time and documentation requirements. Set up a workflow before you start billing them.

4. Code complexity accurately

Primary care visits often address several chronic conditions. Office visit levels are based on medical decision making or total time, and visits that manage multiple conditions may support higher levels than habitually billed. Periodic coding reviews catch undercoding.

5. Vaccines and injections

Bill the vaccine or drug product and its administration. For children, practices in the Vaccines for Children program follow separate rules for state-supplied vaccine. Keep lot numbers and administration documentation complete.

6. Check eligibility every visit

Primary care patients change plans often, especially in Medicaid managed care. Verify coverage and confirm that the patient's assigned primary care provider is your practice for plans that require it.

7. Watch for common denials

  • Preventive visits billed more often than the plan allows
  • Missing or unsupported modifier 25
  • Patient assigned to another primary care provider
  • Terminated coverage
  • Diagnosis codes that don't support medical necessity

8. Collect patient balances

High-deductible plans mean patients owe more. Estimate and collect at check-in, and offer online payment for the rest.

Our family medicine billing team handles these details every day. See our billing service or request a billing audit.

Frequently asked questions

Can I bill a physical and a sick visit on the same day?

Often yes, when a significant, separately identifiable problem is addressed. The problem visit is billed with modifier 25 and must be documented separately.

Is the Medicare annual wellness visit the same as a physical?

No. It's a separate Medicare benefit focused on prevention planning, with its own requirements and codes.

What causes the most denials in family medicine?

Eligibility issues, preventive frequency limits, modifier 25 problems and patients assigned to a different primary care provider.
