How to Establish a Rural Health Clinic in Louisiana

By the HealthRescue Partners team ยท Updated

Rural health clinics receive special Medicare and Medicaid payment to keep primary care available in underserved rural communities. Louisiana has many rural parishes where an RHC can be both a community asset and a sustainable practice. Here's what it takes.

1. Confirm your location qualifies

Federal rules require an RHC to be located in a rural area (based on Census Bureau definitions) that is also a designated shortage area, such as a Health Professional Shortage Area or Medically Underserved Area (based on HRSA designations). Check both before signing a lease. Designations change, so verify them for the exact address.

2. Plan your staffing

  • A nurse practitioner, physician assistant or certified nurse-midwife must be available to furnish patient care at least 50% of the clinic's operating hours. An existing clinic that can't hire one may request a temporary one-year waiver.
  • A physician must provide medical direction, as an owner, employee or under contract.
  • Louisiana NPs work under a collaborative practice agreement, so plan the physician relationship early.

3. Set up the business

Form your entity, get an EIN and group NPI, arrange malpractice and liability coverage, and obtain local business licenses. If you'll run any lab tests on site, you'll need a CLIA certificate.

4. State licensing and survey

The Louisiana Department of Health acts as the state survey agency. You submit the federal Request to Establish Eligibility (Form CMS-29) with supporting documents, meet state licensing requirements, and prepare for an on-site survey of your policies, staffing and facility. CMS's regional office makes the final eligibility determination. Some clinics use an approved accrediting organization instead of a state survey.

5. Policies and compliance

Prepare written policies for patient care, medical emergencies, drug storage, infection control, records and quality review, along with HIPAA documentation. Surveyors check that these exist and are followed.

6. Enroll with Medicare, Medicaid and commercial payers

Enroll as an RHC with Medicare through your Medicare Administrative Contractor, then with Louisiana Medicaid and its managed care plans, then with commercial payers. RHC billing differs from standard physician billing, so set up your billing system and staff training before you open.

Timeline

From choosing a site to receiving certification commonly takes several months, and payer enrollment can continue after that. Start credentialing as early as the rules allow.

Our practice startup and provider enrollment teams can manage the paperwork while you focus on staffing and patient care.
